NAD+ itself and NR are generally sold as dietary supplements; NMN's U.S. supplement status is contested (the FDA has taken the position that NMN is excluded from the dietary-supplement definition because it was investigated as a drug). IV NAD+ is offered by wellness/drip clinics but is not an FDA-approved therapy. None of these is FDA-approved to treat, prevent, or reverse aging or any disease.
SS-31 (Elamipretide)
FDA-approved as Forzinity (elamipretide HCl) — granted accelerated approval on September 19, 2025 for the treatment of Barth syndrome in adult and pediatric patients weighing at least 30 kg; this is the first approved therapy for that ultra-rare genetic mitochondrial disease. As an accelerated approval, continued approval may be contingent on confirmatory trials. It is NOT approved for any other condition: its Phase 3 trial in primary mitochondrial myopathy (MMPOWER-3) did not meet its primary endpoints, and its development in other conditions (e.g., heart failure, dry age-related macular degeneration) has been largely unsuccessful. The 'SS-31' sold online for anti-aging, energy, and 'mitochondrial repair' is gray-market / 'research only' material, not the approved drug, and those uses are unstudied in humans. A mitochondria-targeted tetrapeptide that binds cardiolipin on the inner mitochondrial membrane. Status varies by country and can change; this is not medical or legal advice.
Key differences
- SS-31 (elamipretide) is FDA-approved (Forzinity) for Barth syndrome (S for that narrow use); NAD+/NMN/NR are supplements with an S+ grade only for raising NAD+ levels.
- For anti-aging and energy in healthy people, both are graded B — the marketed benefits aren't established for either.
- SS-31's broader mitochondrial-disease trials largely failed (C); NAD+'s downstream health claims are consistently graded B despite it reliably raising NAD+.
The bottom line
Each has exactly one legitimate, narrow data point — an approval for a rare disease (SS-31) or a measurable biomarker rise (NAD+) — and neither has proven the anti-aging benefits it's sold for. We don't recommend either; we separate the one real finding from the marketing.
