NAD+ itself and NR are generally sold as dietary supplements; NMN's U.S. supplement status is contested (the FDA has taken the position that NMN is excluded from the dietary-supplement definition because it was investigated as a drug). IV NAD+ is offered by wellness/drip clinics but is not an FDA-approved therapy. None of these is FDA-approved to treat, prevent, or reverse aging or any disease.
MOTS-c
Not FDA-approved and not approved by any major regulator for any use. MOTS-c is a naturally occurring 16-amino-acid peptide encoded within the mitochondrial genome (the 12S rRNA gene); as a therapeutic it is investigational. Most evidence is preclinical (cells and animals) plus human biomarker studies; an early-phase (Phase 2a) human trial in adults with prediabetes and overweight/obesity is underway, but no large completed human efficacy trials exist. Synthetic MOTS-c sold online for fat loss, performance, or anti-aging is gray-market / 'research only' material, and those uses are not established in humans. Status varies by country and can change; this is not medical or legal advice.
Key differences
- NAD+/NMN/NR earns an S+ grade for one narrow fact — raising blood NAD+ levels — but B for every downstream health claim; MOTS-c has A-grade preclinical metabolic data but B for any human effect.
- NAD+ precursors are largely sold as supplements; MOTS-c is investigational, with an early Phase 2a human trial only now underway.
- For the fat-loss, performance and anti-aging marketing, both land at B — the human proof isn't there for either.
The bottom line
NAD+ precursors reliably do one measurable thing (raise NAD+) that hasn't translated into the promised benefits; MOTS-c has striking animal data but essentially no human efficacy yet. We don't recommend either — the shared story is a strong mechanism and weak human outcomes.
