Regulatory status at a glance

MOTS-c

Not FDA-approved and not approved by any major regulator for any use. MOTS-c is a naturally occurring 16-amino-acid peptide encoded within the mitochondrial genome (the 12S rRNA gene); as a therapeutic it is investigational. Most evidence is preclinical (cells and animals) plus human biomarker studies; an early-phase (Phase 2a) human trial in adults with prediabetes and overweight/obesity is underway, but no large completed human efficacy trials exist. Synthetic MOTS-c sold online for fat loss, performance, or anti-aging is gray-market / 'research only' material, and those uses are not established in humans. SS-31 (Elamipretide)

FDA-approved as Forzinity (elamipretide HCl) — granted accelerated approval on September 19, 2025 for the treatment of Barth syndrome in adult and pediatric patients weighing at least 30 kg; this is the first approved therapy for that ultra-rare genetic mitochondrial disease. As an accelerated approval, continued approval may be contingent on confirmatory trials. It is NOT approved for any other condition: its Phase 3 trial in primary mitochondrial myopathy (MMPOWER-3) did not meet its primary endpoints, and its development in other conditions (e.g., heart failure, dry age-related macular degeneration) has been largely unsuccessful. The 'SS-31' sold online for anti-aging, energy, and 'mitochondrial repair' is gray-market / 'research only' material, not the approved drug, and those uses are unstudied in humans. A mitochondria-targeted tetrapeptide that binds cardiolipin on the inner mitochondrial membrane. Status varies by country and can change; this is not medical or legal advice.

Key differences

  • SS-31 (elamipretide) is now FDA-approved (Forzinity) for the ultra-rare Barth syndrome (graded S for that use); MOTS-c is investigational, with no approval.

The bottom line

SS-31 has a real approval, but only for one rare disease, and it failed its broader trials; MOTS-c is earlier still — strong in animals, unproven in people. We don't recommend either; the anti-aging marketing for both rests on lab data, not human results.
